Old Insurance A/R Recovery Services Against The Clock.

An inherited backlog is a race with filing and appeal deadlines. We sort every claim by its remaining time, value and next action, work the ones that still have a path, and tell you plainly which ones do not.

  • Separate from your ongoing billing
  • Fees only on insurance payments received
  • BAA before any patient-data access

Two Clocks Decide What Is Recoverable.

Every old claim is either still inside its filing window, inside an appeal window, or out of time. Knowing which, claim by claim, is the first job.

Getting The Claim In

Medicare12 months from the date of service
Commercial plansSet by contract, often much shorter
MedicaidSet by each state

Challenging A Denial

Medicare redetermination120 days from the notice
Medicare reconsideration180 days from the decision
Commercial and MedicaidSet by each plan

Proof of timely filing can rescue a claim the payer says it never received. Clearinghouse acceptance reports showing the original submission date are the evidence, so we look for them first.

Every Claim Gets One Next Step.

No response from the payer

Check status; resubmit with proof of timely filing if it was never received.

Denied, correctable

Fix the data or code and send a corrected claim, not a duplicate.

Denied, records needed

Request the record from your team and send it with the claim or appeal.

Denied, appealable

Write the appeal with the policy, record and argument, inside the window.

Paid to the patient or misposted

Reconcile payments and adjustments before chasing the payer.

Out of time

Close it out with the reason, so it stops sitting in your A/R.

What counts as old A/R: inherited insurance claims older than 120 days when we start. Inherited claims up to 120 days old are worked by a dedicated A/R expert, and claims we submit ourselves stay part of your regular service, even if they age.

What Can Limit Recovery

A missing record, an expired appeal right or a closed filing window can limit what is recoverable. Some claims will not be payable, and we tell you which ones and why instead of working them indefinitely.

Payers make the final coverage and payment decisions.

How Fees Work For Old A/R

  • Its own percentage rate, set in your written quote
  • Applies only to recovered insurance payments you actually receive
  • Replaces your standard rate for that work, never added on top
  • No fee on patient payments

Questions Before You Book.

How Old Is Too Old?

It depends on each payer's filing and appeal limits, and on whether proof of timely filing exists. We review the claims before telling you what is workable.

Do We Need Your Ongoing Billing Too?

No. Old A/R Recovery can be its own engagement or run alongside Medical Billing or Full RCM.

What Does Our Team Provide?

Approved access, the claim history and clearinghouse reports you have, and clinical records when a claim needs them.

Will You Recover Every Claim?

No one can promise that. We work the claims that have a path and report what was recovered and what was closed, with reasons.
